The Adult ADHD Self-Report Scale (ASRS v1.1) was developed with the World Health Organisation. The first 6 questions are the strongest predictors of ADHD; the rest add useful context.

Heads up: This is a screening tool, not a diagnosis. A formal ADHD assessment is the only way to confirm or rule out ADHD.
